| Root cause analysis (RCA) is a multi-disciplinary team approach whose goal is to investigate and incident find out what happened, why it happened and to determine what can be done to prevent it from happening again. Typically it starts with a period of careful, systematic data collection about the incident and the circumstances leading up to it. The information collected is then analysed using a number of simple tools to find causal and contributing factors, and recommendations can be made to prevent recurrence. |
